Housing Stabilization Services is a Minnesota Medical Assistance benefit that aims to help people that are homeless, people with disabilities (including those who are dealing with mental illness and substance use disorder), and seniors find and keep housing.
Housing Consultation Services
We help those who do not have Medical Assistance (MA) and we work with case managers to develop a person-centered plan that addresses their needs, wants, and goals for living in the community. This may include:
- Developing a housing-focused person-centered plan based on the person’s needs, wants, and goals for housing.
- Offering resources related to non-housing goals.
Housing Transition Services
We help people plan for, find, and move into housing. Housing transition services may include:
- Helping with the housing search and application process.
- Developing a budget for the person and helping them understand a lease.
Housing Sustaining Services
We help people keep their housing after they have moved in. This service may include:
- Educating and training on how to be a good tenant; lease compliance, meeting responsibilities, and developing a good relationship with the manager. All to maintain housing stability.
